If you submit our Subcontractor Prequalification Form, we collect the business information you choose to provide, including company and contact details, licensing, insurance, safety and OSHA records, bonding and banking references, financial capacity, project references, legal and claims history, and any documents you upload (such as a W-9, certificate of insurance, licenses, financial statements and OSHA 300A summaries). Uploaded documents may contain a tax identification number or other sensitive business information, so please upload only what we request. Do not send Social Security numbers, bank account numbers, card numbers or passwords through this form.

We use this information solely to evaluate your company for subcontracting opportunities, verify the information you provide, and communicate with you. Your submission is delivered by email to our internal team and is accessed only by personnel who need it for that purpose. We use a third-party email delivery service to transmit your submission, and we do not sell or rent it. We may contact the references, insurance agents, bonding companies and banks you list in order to verify your information, and by submitting the form you authorize us to do so.

We keep prequalification submissions for as long as needed to evaluate and manage a subcontracting relationship and to meet legal, insurance and recordkeeping obligations, and we then delete them in the ordinary course of business. You may request access, correction or deletion at any time using the contact details in Section 10. Please include only business contact details for other individuals, such as your references. Submitting the form does not guarantee approval, an engagement or any work, and the information you provide must be true, complete and accurate.
